About Us

Castle Rock Home Care is a growing provider of pediatric and developmental disability services dedicated to helping children and families access the care they need. We are seeking an experienced and relationship-driven OPWDD Community Marketing Representative to expand our presence throughout New York City and build meaningful partnerships within the developmental disability community.

Responsibilities
  • Develop and maintain relationships with OPWDD Care Managers, CCOs, schools, hospitals, clinics, social workers, and community organizations.
  • Educate referral sources on available home care, nursing, therapy, and support services.
  • Attend networking events, conferences, resource fairs, and community outreach initiatives.
  • Serve as a trusted resource for families navigating OPWDD eligibility and service coordination.
  • Collaborate closely with intake, eligibility, and clinical teams to ensure smooth referral conversion.
  • Maintain accurate referral tracking and pipeline management within CRM systems.
  • Identify new business opportunities and strategic partnerships throughout NYC.
Qualifications
  • Must have active OPWDD care management experience.
  • Experience in home care, healthcare marketing, care coordination, or business development.
  • Strong understanding of the OPWDD system and developmental disability service landscape.
  • Existing relationships with Care Managers, CCOs, schools, or community organizations are preferred.
  • Excellent communication and relationship-building skills.
  • Self-motivated, organized, and comfortable working independently in the field.
  • CRM experience preferred.
Performance Expectations

Successful candidates will be expected to build and manage a robust referral network capable of generating consistent monthly growth. The target for this role is 10 qualified Starts of Care (SOCs) per month
, achieved through community outreach, referral source development, and strategic relationship management.
