Admissions & Intake Coordinator

West Hempstead, NY / On-site

$60,000–$65,000 base salary + performance-based bonus opportunity of up to $30,000 annually

Overview

We’re looking for an energetic, motivated Intake Coordinator to join a growing home care organization during an exciting stage of expansion. This is a high-volume, fast-paced role for someone who enjoys talking to people, building relationships, and following through until the job gets done. Success in this role requires outstanding communication skills, persistence, a strong work ethic, coachability, and the drive to achieve results.

You’ll play a key role in helping patients and families access care while building relationships with referral sources, guiding prospective clients through the admissions process, and ensuring every opportunity is followed through from initial contact to approval for services. Comprehensive training will be provided—we’re looking for the right attitude and motivation more than prior industry experience.

Responsibilities
  • Serve as the first point of contact for prospective patients and their families.
  • Build and maintain strong relationships with referral sources and healthcare professionals.
  • Manage a high volume of inbound and outbound calls.
  • Follow up consistently with prospective patients and referral sources to maximize admissions.
  • Guide patients through the intake process from initial inquiry through approval for services.
  • Verify patient eligibility and collect all required documentation.
  • Coordinate the authorization process and ensure required information is submitted accurately and on time.
  • Keep every referral and opportunity moving by staying organized and persistent.
  • Maintain accurate patient records and document all communication throughout the intake process.
  • Work cross-functionally with the operations team to ensure a seamless transition from intake to care.
  • Take ownership of each case from initial contact through completion, ensuring nothing falls through the cracks.
  • Help build and improve intake processes as the department continues to grow.
Qualifications
  • No prior intake or healthcare experience required—comprehensive training will be provided.
  • Customer service, sales, admissions, administrative, call center, or healthcare experience is a plus.
  • Excellent phone presence and relationship-building skills.
  • Motivated by goals, performance, and achieving results.
  • Persistent, proactive, and comfortable following up until the job is done.
  • Friendly, energetic, and professional with outstanding communication skills.
  • Highly organized with strong attention to detail.
  • Coachable, eager to learn, and quick to adapt.
  • Strong follow-through, accountability, and problem-solving skills.
  • Comfortable learning new systems and technology.
