About Conduit Health
Conduit Health is fixing one of the most outdated corners of healthcare — getting medical equipment and supplies into patients' homes. We've built the first vertically integrated platform that unites ordering, telehealth, prescriptions, insurance, and fulfillment, so a case manager can say "yes" in seconds to a patient who would otherwise wait weeks. We've raised $22M+, tripled in the past year, and are scaling fast.
The RoleYou open the door for Partner Sales: find the home health agencies, hospice providers, and post-acute organizations whose patients wait weeks for equipment, get their intake directors and agency owners on the phone, and book qualified discovery meetings for the Partner Sales Lead.
This is an outbound seat. Most of your day is on the phone and in inboxes. You'll be measured on qualified meetings held — not dials for their own sake, and not meetings that don't show. You'll also help write the outbound playbook and sharpen the ICP as you go, because you'll hear the market more than anyone else on the team.
Do this well and the path to Account Executive or Partnerships is real and close. Our founding AE built their book from nothing; you'll be next in line to do the same.
What You'll Do Outreach — 70%Make 60–100 calls a day to directors of patient services, intake managers, and agency owners
Send 40–60 personalized emails a day inside multi-channel sequences you help build
Work Linked In with intent — connection requests, voice notes, and real comments on prospects' posts
Handle the first objection, explain the problem Conduit solves, and earn the next 30 seconds
Book qualified discovery meetings for the Partner Sales Lead, with clean notes so the call starts warm
Build and refine target lists of home health agencies, hospice providers, and post-acute organizations
Research an account in five minutes, not fifty — personalize enough to land, then dial
Use Sales Navigator, Hub Spot, and Apollo to find the right person and an accurate number
Spot buying signals: new agency openings, leadership changes, census growth, a competitor stumbling
Log every touch in Hub Spot the same day. Clean data isn't optional here.
Bring objections and market feedback back to Sales, Product, and Marketing every week
Turn what's working into the playbook: talk tracks, sequences, and the ICP definition
Ramped on the product, the coverage basics, and the pitch — hitting daily activity targets by week two
A clear point of view on which segments and personas convert, backed by your own numbers
Consistently producing 20–25 qualified meetings a month that hold
Two concrete improvements to the outbound playbook that came out of your own calls
1–2 years in an SDR, BDR, or inside sales role where the phone was your primary channel — healthcare or B2B services preferred, not required
Comfortable s isn't an email-only role, and you don't need to be talked into dialing.
Resilient and competitive. Rejection doesn't derail your day; a bad morning doesn't become a bad week.
A clear writer. Your cold emails sound like a person and would survive an intake director's inbox.
Disciplined about the CRM. Your activity is a record, not a Friday chore.
Curious about healthcare. You want to understand how a patient actually gets a hospital bed at home today, and why it takes so long.
You've sold into home health, hospice, or skilled nursing;
Hub Spot, Apollo, Instantly, Aircall, or Sales Navigator; startup experience where you had to figure it out without a playbook.
Not inbound qualification. You build the list and make the first call.
Not a closing seat yet. You book the meeting; the Partner Sales Lead runs…
