Cooperative Home Care Associates (CHCA) is seeking a Client Enrollment Coordinator to manage the referral and enrollment process for individuals seeking home care services. This position works closely with prospective clients, families, health plans, and referral sources to guide clients through enrollment, address barriers, and ensure timely follow-up.
The ideal candidate is organized, outgoing, customer-focused, and comfortable managing multiple referrals and building relationships within the community.
Key Responsibilities:- Manage prospective clients through the referral and enrollment process from initial contact through enrollment.
- Conduct introductory calls, explain enrollment requirements, and assist clients with required paperwork.
- Maintain regular follow-up with prospective clients, families, health plans, and referral sources.
- Track enrollment activity and maintain accurate documentation in databases and tracking systems.
- Identify and resolve issues that may delay or prevent enrollment.
- Assist prospective clients with Medicaid eligibility and enrollment, including applications and required documentation.
- Stay informed of Medicaid eligibility requirements and follow up with clients regarding changes in coverage.
- Build relationships with health plans, community organizations, and referral partners.
- Represent CHCA at community and outreach events to promote services and generate referrals.
- Support efforts to grow CHCA's client census, including private-pay referrals.
- Perform other duties as assigned.
- Minimum of 2 years of intake or related experience preferred.
- Knowledge of Medicaid eligibility and entitlement programs.
- Bilingual English/Spanish required.
- Strong customer service, communication, and relationship-building skills.
- Excellent organizational, time-management, and follow-up skills.
- Proficiency in Microsoft Office; experience with HHA Exchange, CRM, or similar healthcare systems preferred.
- Comfortable working with diverse communities and populations.
- Self-motivated, professional, outgoing, and results oriented.
- Ability to work independently and collaboratively as part of a team.
- Ability to work occasional weekends or on call as needed.
