Case Manager, Kinship Reentry Program

Osborne Association is dedicated to helping individuals, families, and communities affected by the criminal justice system. We aim to heal, repair, and restore lives while challenging systems rooted in racism and retribution.

We are currently seeking a full‑time Case Manager to join our Kinship Reentry Program in Brooklyn.

Hourly Rate: $28‑$30 per hour.

Responsibilities
  • Provide direct services including assessments, intakes, and supportive counseling for families.
  • Document service activities via Osborne’s database, including case notes, service plans, and home visits.
  • Complete supervision agenda and submit it 24 hours before the meeting.
  • Conduct and schedule routine home visits—up to four per family each quarter (initial visit part of intake).
  • Monitor and document client progress toward service plan goals and maintain detailed individual progress notes.
  • Identify and refer families to appropriate external services and resources.
  • Facilitate orientation and attend the monthly virtual financial literacy workshops for new families (6 pm–7 pm).
  • Prepare for audit procedures as needed.
  • Follow up with families bi‑weekly to assess needs and progress, documenting all interactions.
  • Contribute ideas for program development and innovation.
  • Attend assigned training sessions.
  • Support recruitment and outreach activities.
  • Collaborate in person with colleagues (in‑person attendance required).
  • Perform other duties as assigned.
Minimum Qualifications
  • Bachelor’s degree in criminal justice, social work, or psychology with at least one year of case management experience; or high school diploma with at least three years of progressive case management experience with families and the criminal justice population.
  • Experience working with families and the criminal justice population.
Key Competencies
  • Engaging personality and detail‑oriented skills.
  • Strong oral and written communication, time‑management, and organizational skills.
  • Availability for some evenings and occasional weekends.
  • Proficiency with CARES, MS Word, Excel, Google Suite, and database management.
  • Familiarity with community resources.
  • Problem‑solving, conflict resolution, and crisis‑management abilities.
  • Local travel capability.
  • Collaborative teamwork with diverse groups.
  • Capacity to manage multiple tasks under pressure.
