FLSA Status: Non-Exempt

Pay & Benefits: The pay range for this role is generally $46,300 - $51,300 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

POSITION

SUMMARY

The Domestic Violence & Trauma Services Program offers a comprehensive suite of services to survivors of domestic violence with enhanced focus on education and linkage to health & wellness through both individual and group counseling supportive services. CHASI works in collaboration with the Staten Island Family Justice Center to expand reach and capacity. Domestic violence and trauma services are co-located. Services are provided at two different locations, 26 Bay Street Site and the Staten Island Family Justice Center (FJC).

The Intensive Case Manager will manage a high-risk caseload with a specific focus on working with complex, intersectional cases of domestic and gender-based violence including, but not limited to, family violence, elder abuse, human trafficking, and sexual violence. The Intensive Case Manager will navigate clients through the many complex systems called into action in service plans, such as behavioral health services, medical services, criminal justice, family court, and child protective services.

The Intensive Case Manager will be a strong advocate who can provide ongoing support to clients with the police, prosecutors, housing, shelters, schools, public assistance, and other service providers. The Intensive Case Manager will provide on-going and enhanced follow-up with clients to ensure that clients are accessing and successfully connecting with the array of supportive services available at the FJC and through off site partnerships and linkages, creating positive relationships with on and off-site partners.

DUTIES

& RESPONSIBILITIES
  • Conduct initial comprehensive assessments, safety planning, and risk assessments with clients.
  • Navigate the client to execute their service plans, including making active referrals, accompany clients to various appointments, provide coaching to clients, deliver weekly or monthly education encounters, plan logistics for plan adherence reminders, conduct regular follow-up, and assist with transportation to get to and from appointments. Contact clients via phone, text, and email should they fail to attend appointments or stray from documented service planning.
  • Provide trauma-informed education about domestic violence and all forms of gender-based violence.
