Volunteers of America-Greater New York (VOA-Greater New York) is an anti-poverty organization working to end homelessness in the New York area by 2050. We operate over 60 programs across New York City, Northern New Jersey and Westchester County including emergency, transitional and permanent housing. Our services extend to families experiencing homelessness, survivors of domestic violence, people living with HIV/AIDS, veterans, youth aging out of foster care, adults with behavioral health or substance use needs , and others in need, including a special education preschool program.

Position:
Case Manager
The Case Manager works with the resident to assess their strengths and needs, plan a course of action, link the resident to services, counsel the resident and monitor progress. The case manager must assure that residents receive appropriate, competent, and professional information and advice, referrals and advocacy services to achieve successful outcomes.
Location: Bronx, New York 10453
Minimum Qualifications:
- The position requires a bachelor’s degree or 60 college credits plus two years relevant experience.
- The position also requires two years of experience working with the homeless and/or senior population, a demonstrated ability in verbal and written communication, including basic computer skills.
Case Manager Principal Responsibilities:
- Manage the resident intake and assessment process to orient the resident to the program and produce a comprehensive assessment of resident service needs.
- Develop with the resident a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the resident as required.
- Meet regularly with the resident to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
- Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.
- Provide crisis intervention and emergency response and make the necessary and appropriate referrals and linkages.
