Qualified applicants will need to be approved by NYS DOH.
Help residents achieve and maintain optimum social, mental, spiritual, and emotional well‑being. Serve as a resource for residents and their families. Oversee all record‑keeping practices and ensure they are effective and efficient.
Qualifications- Collegiate degree and related experience, specifically:
- A Bachelor’s or Master’s degree from an accredited college or university in Human Services, Case Management, Social Work or the equivalent, and one year of full‑time experience working with the adult or geriatric population; OR
- An Associate Degree from an accredited college or university in Human Services, Case Management, Social Work, or the equivalent, and three years of full‑time experience working with the adult or geriatric population.
- LPN or RN license
- Ability to work flexible hours and weekends
- Strong organizational and time‑management skills
- Ability to respond calmly to emergencies
- Good judgment, problem‑solving and decision‑making skills
- Successfully complete training as required by Meridian in accordance with state regulations
- Ability to communicate effectively with residents, families, staff, vendors and the general public
- Compassion for and desire to work with the elderly
- Ability to work responsibly as a team member as well as an individual
- Honesty, fairness, dependability, respect for confidentiality and the rights and privacy of others, and knowledge of local, state and federal laws
- Practice and promotion of Meridian Policies and Procedures, Mission Statement, Core Values and Founding Principles
- Meet all health requirements and pass background checks
- Perform duties and responsibilities (essential job functions), with or without reasonable accommodation
