Status:
Full‑time (35 hours per week)
- Maintain professional relationships with clients and uphold client confidentiality.
- Practice Universal Precautions / Standard Protocol & Procedures.
- Comply with all federal, state, city, and CAMBA security and privacy policies protecting individually identifiable health information.
- Review documentation establishing clients’ eligibility for the program and make file copies.
- Create and maintain client files, input data and progress information into the CARES database.
- Conduct initial intake and psycho‑social assessment of clients and families, and perform periodic re‑assessments.
- Collaborate with clients to prepare initial and periodic revisions of independent living plans, short‑ and long‑term goals.
- Assist clients in attaining goals by identifying and locating community resources.
- Refer clients to permanent housing opportunities and appropriate services within and outside CAMBA.
- Schedule appointments with referral organizations and escort clients to appointments as needed.
- Assist clients in completing applications for benefits and entitlements, or process applications on their behalf.
- Work with clients to overcome barriers, advocate for themselves, and move toward self‑sufficiency.
- Recommend and implement strategies to increase client participation.
- Monitor clients’ progress via scheduled telephone contact, face‑to‑face visits, and document progress notes.
- Follow‑up with clients and referral organizations regarding contact and progress.
- Act as client liaison/advocate with outside organizations concerning education, health care, housing, legal issues, etc.
- Provide required information for weekly/monthly/quarterly/annual reports.
- May recommend closing of cases when primary goals are achieved and stability is maintained, or when the client demonstrates unwillingness to participate, or becomes ineligible.
- May follow‑up after successful completion to assure stability.
- May complete Relocation Case Review for referral of after‑care services.
- May prepare marketing materials for the program.
- May prescreen clients by phone for eligibility and schedule intake appointments.
- Tasks may be modified, expanded, or assigned over time.
- Bachelor’s degree (e.g., B.A., B.S.W.) or
- Four years of relevant experience, or
- Associate’s degree and two years equivalent experience.
- Bidirectional fluency in English and Spanish.
Ability to obtain and maintain the following clearances throughout the duration of employment:
- New York State Division of Criminal Justice Services (DCJS)
- State Central Registry (SCR)
- Register of Substantiated Category One Cases of Abuse or Neglect (Staff Exclusion List)
- Proof of negative tuberculosis test results.
Annual salary: $44,800–$51,500.
