This is a full-time position which includes one evening/late shift (11:00 AM - 7:00 PM) and weekend coverage. Lantern staff may be asked to adjust their schedule and hours based on the operational needs of the site and the agency.

Where You Will Work: Hudson Bay is a brand-new, all-female shelter in the Bronx housing 200 women aged 55 and older.

About the Role

About the Role: We are seeking experienced Case Managers for our new women’s shelter in the Bronx. Case Managers are part of an interdisciplinary team focused on stabilizing shelter clients by strengthening independent living skills and supporting clients in achieving their housing goals. Case Managers are responsible for conducting assessments, linking clients to appropriate services, developing and implementing service plans, supporting stabilization and re-housing efforts, monitoring progress, and documenting all client interactions.

The successful candidate will have experience working with individuals experiencing homelessness and/or mental health challenges.

Essential Functions
  • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
  • Conduct intake and assessments to orient clients to the program and develop individualized service plans based on identified needs and goals.
  • Meet weekly with clients to support progress toward service plan goals.
  • Coordinate services with the Housing Specialist, shelter staff, and external partners.
  • Advocate for clients regarding benefits, entitlements, and service access.
  • Provide crisis intervention and supportive counseling to strengthen clients’ decision-making and coping skills.
  • Document all case management activities in accordance with agency and funding guidelines.
  • Complete housing applications (both written and electronic) based on clients’ needs and eligibility.
  • Achieve assigned monthly housing placement quotas. Facilitate workshops and groups, and escort clients to appointments as needed.
  • Conduct weekly or biweekly unit or dorm inspections.
  • Attend staff meetings and trainings to enhance job-related skills.
  • Work flexible schedules, including evening and weekend shifts as required.
  • Perform other related duties as assigned.
Required

Education and Experience
  • High school diploma, HSE, or GED required.
  • Minimum of two (2) years of social services experience required.
  • Valid Food Handler’s License or ability to obtain within six (6) months of hire required.
  • Experience working with individuals experiencing homelessness, mental illness, substance use, and/or low-income populations.
  • Strong relationship-building and communication skills required.
  • Strong computer skills required, including Microsoft Word and Excel.
  • Ability to work evenings and/or weekends on a rotating schedule.
  • Prior experience working in a homeless shelter preferred.
  • Knowledge of mental health, substance use, and homelessness service systems preferred.
Work Environment

Work Environment: This position operates in a shelter environment and works with a population who may experience mental illness, substance use, disability, HIV/AIDS, and other chronic health conditions.

Physical Demands

Physical Demands: While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally, the employee may lift and/or move up to 10 pounds.
