Case Coordinator

Hourly Full-Time [Ben Elig] Office/Clerical – The Bronx, NY, US

Salary: $59,410 per year

Hours:

Monday – Friday, 9:00 AM – 5:00 PM (35 hours per week, excluding breaks)

Location:

424 East 147th Street, Bronx, NY 10455 – accessible by public transportation

Workplace Flexibility:
Hybrid – at least one full day remote per week

Job Summary

CASES is seeking a Case Coordinator for our Bronx Pretrial Services team. The Case Coordinator conducts Supervision Introduction/Orientation meetings shortly after participants are released into the Supervised Release Program (SRP). The role involves ensuring participants comply with supervision appointments and court appearances, conducting ongoing service need assessments, and developing collaborative service plans that address identified needs and goals. The Coordinator supports participants in achieving self‑identified treatment, recovery, rehabilitation, and crime‑free community integration goals while maintaining the core principles of risk, need, and responsivity.

Responsibilities
  • Provide supervision and community‑based services to a caseload of pretrial participants
  • Within 24‑48 hours of release, conduct Supervision Introduction/Orientation meetings and review supervision conditions, reporting schedules, and communication methods
  • Collaborate with the court and pretrial staff to facilitate participants’ completion of mandated supervision appointments and court appearances
  • Analyze legal and court screening documentation with participants to identify offending behavior patterns and potential interventions
  • Conduct ongoing service need assessments to evaluate psychosocial needs, substance use disorders, mental health issues, and motivation for treatment
  • Link participants to appropriate treatment and services, providing escort to community resources when necessary
  • Maintain the flow of community outreach and case management activities to meet program goals and objectives
  • Apply problem‑solving techniques and motivational interviewing to engage participants in necessary changes
  • Provide services in a gender‑responsive, trauma‑informed manner consistent with risk‑need‑responsivity principles
  • Develop and sustain collaborative relationships with community providers (housing, health homes, addiction treatment, mental health, peer‑run services, educational, vocational, legal, medical, and family providers)
  • Follow monitoring protocols for high‑risk participants and engage in case reviews to address complex cases
  • Enter assessment data, supervision appointments, and progress notes into the program database, ensuring confidentiality and timeliness
  • Meet monthly case‑management contact and outreach goals set by supervisory staff
  • Provide culturally competent services in accordance with CASES policies and practices
  • Perform additional duties as required by the Program Director and supervisory staff
Qualifications
  • Bachelor’s degree with 1‑2 years of experience, or an associate’s degree with a minimum of three years of experience in human services with individuals who have substance use disorders, co‑occurring mental health and substance use disorders, and criminal justice involvement
  • Extensive knowledge of community treatment, support services, and resources
  • Knowledge of the criminal justice system, court processes, and alternatives to detention/incarceration services
Preferred Skills
  • Experience using databases such as Salesforce
  • Fluency in Spanish
