POSITION SUMMARY:

Under the direction of the Director of Social Services, the CMS is responsible for the daily oversight and performance of eight case managers in a men emergency shelter. The position will ensure the delivery of quality services to all clients on a 24-hour basis as per CPU and NYC Department of Homeless Services (DHS). The Case Work Supervisor will work closely with Director of Social Services to provide support co Case Managers in coordinating and delivering high quality of care to people served.

POSITION

DUTIES:
  • Assign cases to Case Managers and overseeing their work to ensure that they are meeting the standards of performance correctly and efficiently.
  • Ensure that ILP are conducted bi-weekly and that clients are seen on a weekly basis.
  • Provide staff training on areas of their work necessary for good delivery of services.
  • Arrange for case presentations and assisting case managers with case selection
  • Hold morning huddles with case managers to review and plan for corrective plan of action.
  • Schedule work according to priorities.
  • Hold individual and group supervision with the Case Manager’s team.
  • Provide individual counseling, advocacy and support to clients when necessary to review cases and address problems.
  • Conduct regular case reviews to ensure files are in compliance
  • Whenever possible, engage in community outreach efforts to identify housing opportunities.
  • Develop and maintain relationships with government partners, and supportive community-based organizations.
  • Coordinating services with other agencies to ensure that clients receive the care they need.
  • Serve as a resource for the shelters’ social services department staff.
  • Represent CPU at city and statewide coalitions, task forces and committees as necessary.
  • Participate in monthly staff meetings, case conferences and training seminars as indicated by Director of Social Services
  • Assist with reports and program evaluations.
  • Perform other duties as assigned by the Director of Social Services.
  • Interprets regulations, policy and changes for the Case Managers
Requirements:
  • Bachelor’s degree in human services with two or more years of experience working with disenfranchised communities or master’s degree in human services or related field with at least one year of experience.
  • Knowledge and skills to develop strong linkages with city and community-based services in order to secure services timely for clients.
  • Knowledge of housing programs and opportunities for single male, preferred
  • Must have the ability to be flexible with schedule as needed.
  • Must be computer-literate and must have knowledge of Microsoft Office applications.
  • Strong computer skills including Microsoft Word, excel and Power Point.
  • Knowledge of CARES DHS System preferred.
