Central Post-Acute Resource Center (PARC) Specialist – Columbia
New York-Presbyterian is committed to strengthening post‑acute care through collaboration with community resources. The Central PARC Specialist will support the post‑acute care strategy, working under the supervision of the Central PARC Program Manager to improve care transitions and enhance patient outcomes.
Responsibilities• Strengthen care transitions across the continuum and develop relationships with post‑acute care providers.
• Identify and implement strategies that improve the post‑discharge experience and outcomes for patients.
• Foster and maintain effective partnerships with post‑acute providers and ensure clear communication among key stakeholders.
• Enable care teams to facilitate efficient discharges.
• Conduct follow‑up calls with patients and periodic site visits to post‑acute care facilities to identify shared opportunities for improvement and support seamless care transitions.
• The position is Monday‑Friday; weekends and holidays are on rotation.
- Master’s Degree
- Clinical background (RN, LMSW, LCSW)
- Experience working with post‑acute care providers (including discharge planning, care coordination and knowledge of the continuum of care planning process)
- Minimum of 5 years in a healthcare environment
- Bachelor’s Degree
- Basic knowledge of performance improvement concepts and EMR documentation
$74,000–$111,000 Annual
