Title: Case Manager

Salary Range: $42,290-$46,290

Program Overview

The Barbara Kleiman Men’s Shelter is a 200-bed, 24/7 emergency shelter for homeless men. The program serves individuals diagnosed with mental illness and/or substance use disorders (130) and those employed or seeking employment (70). Services include case management, individual and group counseling, mental health care, primary medical care, recreational activities, occupational therapy, employment services, job readiness services, and housing placement assistance.

Position Overview

Under the general direction of the Director of Social Services and the Team Leader, the Case Manager provides assessments, case management, and service delivery to clients. Responsibilities include assisting clients in addressing their dependencies, enabling them to gain control over their lives, and helping them transition to permanent or alternative housing. The Case Manager also offers referral services, ensures protocol adherence, maintains audit-ready charts, and performs related duties as required.

  • The essential duties of the Case Manager include but are not limited to the following activities:
  • Conduct intake and assessments, make necessary referrals, and complete/update psycho-social assessments of clients
  • Provide case management services and help clients develop and adhere to Independent Living Plans by identifying short- and long-term objectives
  • Input all client information and update it in a timely manner in the CARES system and the client’s physical chart
  • Work collaboratively with clients and staff to secure financial entitlements, housing, and other concrete services
  • Maintain accurate and up-to-date written client documentation as required and in accordance with City/State oversight agency guidelines
  • Guide clients through individualized sessions on issues such as achieving personal goals, securing housing, maintaining sobriety, complying with medication protocols, and pursuing employment opportunities
  • Maintain liaison with representatives from community organizations
  • Provide escort services to clients’ appointments as needed
  • Attend and participate in program meetings
  • Perform other duties as assigned by the Team Leader, Director of Social Services, and/or Program Director
Physical Activities

The Case Manager must navigate the facility by foot, climb stairwells, observe staff and clients, stand and bend to review client files, operate a computer workstation for extended periods, and lift or move material and equipment up to 15 pounds.

Qualifications
  • A bachelor’s degree in social work, public administration, psychology, education, sociology, occupational or physical therapy, or other related human services fields with a minimum of two (2) years of experience
  • An associate’s degree in the aforementioned fields with three (3) years of experience
  • A high school diploma or GED equivalent with four (4) years of experience
  • Bilingual or multilingual skills are a plus

  • Leadership: Demonstrated ability to manage and motivate staff to accomplish program goals while developing individual and group skills
  • Team building: Ability to develop and foster team spirit, enabling staff to overcome challenges in serving a special client population
  • Communication: Excellent oral, writing, and listening skills are required
  • Organizational: Ability to work well within a highly pressured, deadline-driven environment while meeting program mandates
  • Interpersonal: Ability to interface with clients and all levels of staff
  • Computer: Knowledge of case management software and proficiency in Microsoft Office Suite
