Care For the Homeless (CFH)

is a nationally recognized nonprofit leading the way in caring for those experiencing homelessness and housing instability in New York City. Since 1985, as the city’s first “Health Care for the Homeless” program funded by the Robert Wood Johnson Foundation and Pew Charitable Trusts, we have integrated healthcare, shelter, and social services in a coordinated, innovative model of care that meets people where they are.

By providing comprehensive, compassionate support across 16 community health centers and five shelters throughout the city, we improve health outcomes and create pathways to stability. Our work is grounded in dignity, equity, and the belief that every New Yorker deserves access to quality care and the opportunity to live a healthy, stable life. We are proud to help end episodes of homelessness every day.

Summary

Reports to the collaborating physician at the contracted facility.

Responsibilities
  • Participates as a member of committees, including peer review, as requested by the CMO
  • Chart completion is consistent with CFH P/P
  • Reaches out and engages clients with appropriate concern for their behavioral health care needs
  • Coordinates medical care with PCP for chronic and acute non-behavioral health conditions
  • Advises and counsels clients about their physical and mental health; coordinates care with case management and health education as clinically indicated
  • Provides consultation and training to staff and volunteers where appropriate.
  • Assists with the identification, counseling, and care coordination, with case management and health education, of mental health and substance abuse problems.
  • Works in a non-traditional environment; e.g., a non-clinic facility where homeless people congregate.
Qualifications
  • New York State Nurse Practitioner License
  • Commitment to serving the underprivileged
  • DEA registration.
  • Flexible and ability to function in a nontraditional setting.
