About Queens Community House (QCH)
Queens Community House (QCH) is a multi-site nonprofit organization serving the diverse population of Queens through a broad network of programs for children and youth, adults and families, and older adults. As a modern interpretation of the traditional settlement house, QCH brings innovative leadership and a strong commitment to social and economic justice to its work.
QCH aims to transform the Pomonok Community Center (PCC) into a model settlement house, providing holistic community engagement and human services to improve the quality of life for Pomonok residents. With Pomonok Houses home to many multigenerational families, advancing economic mobility and long-term family stability is a key priority. This initiative is a high priority for QCH leadership and serves as a model for deeper community engagement across the organization.
Position Overview The Family Support Coordinator (FSC) is a key member of the Pomonok team and plays a critical role in advancing the goals of the Pomonok initiative. The FSC builds strong, meaningful relationships with residents and supports households in achieving greater stability.
As an engaged and accessible presence in the community, the FSC listens to residents’ concerns and serves as a trusted connector to services, resources, and opportunities available through the Pomonok Community Center and Queens Community House. This role involves conducting assessments, developing family-centered action plans, and connecting households to essential supports such as benefits enrollment, housing assistance, mental health services, and financial partnership with the Youth Services Student Coordinator and outreach staff at nearby PS 201, the FSC also supports parent and caregiver engagement, strengthens school‑family connections, and helps address emerging family needs.
Key Responsibilities Build and maintain strong, trusting relationships with residents, serving as a community-facing connector to services and resources through the Pomonok Community Center, PS 201, and QCH programs
Conduct comprehensive household assessments and provide integrated, strength-based case management
Work collaboratively with individuals and families to identify priorities, assess needs, and develop actionable plans that support stability and well-being
Maintain a regular on-site presence within the Pomonok community; participate in occasional evening and weekend activities
Connect families to QCH programs and external resources, including public benefits, housing assistance, mental health services, financial counseling, education, and workforce programs
Support parent and caregiver engagement at PS 201 in collaboration with QCH Youth Services staff
Facilitate community engagement and cross-program collaboration at the Pomonok Community Center Participate in cross-department meetings and case conferences to ensure coordinated service delivery
Maintain accurate participant records and track engagement, milestones, and outcomes using a data-driven approach
Contribute to creating a welcoming, supportive, and inclusive environment for residents
Required Qualifications MSW preferred (with 2–3 years of relevant experience), or bachelor’s degree in social work, human services, or a related field with 5+ years of case management and community engagement experience
Demonstrated ability to conduct comprehensive assessments and navigate sensitive conversations
Strong collaboration skills with families, schools, community partners, and internal teams
Ability to work independently with a high degree of autonomy while maintaining accountability for outcomes
Excellent communication and interpersonal skills
Strong organizational skills and attention to detail; ability to manage multiple cases simultaneously
Commitment to strength-based, family-centered, and community-driven work
Preferred Qualifications Knowledge of community resources, housing systems, and public assistance programs
