Description:

Concern Housing is seeking a Substance Use Case Manager who will be responsible for providing case management and chemical dependency counseling services directly to clients receiving Mobile Residential Support Team services.

Responsibilities Include but are not limited to:

  • Provision of chemical dependency counseling services to assist individuals to develop the skills needed to prevent relapse, promote recovery and independence and increase self-sufficiency.
  • Provision of case management services (assistance in forming linkages and/or referrals to clinical services, day programs, medication management, life skills counseling services vocational services, and opportunities, entitlements etc.)
  • Timely submission of all documentation, and maintenance of all client charts in compliance with agency standards.
  • Attendance at supervision and staff meetings.
  • Other duties as assigned by the Director of Supported Housing.
  • Overall assessment of client welfare and appropriate intervention and follow-up as needed.
  • Remain current with all Agency required compliance documents and trainings.

Position Requirements: CASAC or CASAC-T credential is required for this position. Bachelor’s Degree in Psychology, Rehabilitation, Social Work or other related discipline, plus 2 years’ experience working in a similar setting and/or working with people in recovery from chemical dependence.
