Salary Range: $42,000 - $46,000 per year

Description:
The New Providence Women’s Shelter is a 130‑bed 24/7 emergency shelter for homeless women diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, recreational activities, and housing placement assistance. Additionally, an on‑site Article 28 Clinic provides primary and behavioral health care services.
- Conducts intake and assessments, makes necessary referrals for services, and completes/updates psycho‑social assessments of clients.
- Provides case management services and assists clients in the creation of and adherence to their Independent Living Plans by identifying short and long‑term objectives that can be achieved.
- Inputs all client information and updates it in a timely fashion into the CARES system as well as the client’s physical chart.
- Works collaboratively with clients and other staff to identify and obtain financial entitlements, housing, and other concrete services.
- Maintains accurate and up‑to‑date written client documentation as required and in accordance with City/State oversight agency guidelines.
- Provides guidance for clients through individualized sessions on issues such as achieving personal goals, finding permanent or alternative housing, maintaining sobriety, complying with medication protocols, and pursuing employment opportunities.
- Maintains liaison with representatives from community organizations.
- Provides escort services to clients’ appointments as needed.
- Attends and participates in program meetings.
- Performs other duties as assigned by the Team Leader, Clinical Director, or Program Director.
While performing the duties of this position, the Case Manager must stand to talk to clients and staff, sit for extended periods at the computer, and reach or squat to retrieve records from files. The Case Manager must also be able to use the New York City Transit system to escort clients to scheduled appointments, interviews, or meetings in any of the five boroughs.
Qualifications- High School/GED Diploma and experience as described below or Associate’s Degree.
- One to two years (1‑2) of experience working with homeless adults, substance use, and/or mental health populations.
- Teamwork, organizational, interpersonal, communication, computer skills; bilingual a plus.
- Ability to work cooperatively with peers and other staff to serve the needs of clients effectively and efficiently.
- Ability to work independently and adjust one’s schedule to accommodate client needs.
- Excellent oral, writing, and listening skills.
- Ability to interface with clients as well as all levels of staff.
- Knowledge of case management software (CARES); proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.
- Bilingual or multilingual a plus.
