Community Housing Innovations (CHI) is a not‑for‑profit housing and human services organization that supports social and economic independence. We are seeking a Part Time Case Manager to provide supportive case management services to tenants in our permanent affordable supportive housing program.

Job Summary

Position:
Part Time Case Manager

Hours:

20 per week – Monday, Friday & Saturday 9am‑2pm;
Tuesday 2pm‑7pm (off Sunday, Wednesday‑Thursday).

Location:

Eastchester neighborhood, Bronx, NY.
Pay: $24.04 hourly; approx. $25,376 annually. FLSA: non‑exempt; eligible for overtime.
PS:
The role supports 81 subsidized apartments within a 116‑unit multi‑dwelling complex.

Responsibilities
  • Screen DHS referrals for eligibility and certification for program enrollment.
  • Conduct tenant intake and needs assessment to develop independent living plans (ILPs). Track progress and milestones.
  • Provide case management according to ILPs; coordinate workshops or trainings to promote independent living (budget management, health, nutrition, housekeeping, vocational/employment skills).
  • Engage tenants in supportive and service programs such as Workforce Development and Financial Literacy; refer to community‑based services.
  • Advocate on behalf of tenants for entitlements, benefits, employment opportunities, and community services.
  • Link tenants to employment support services (resume building, interview skills, job skills).
  • Ensure timely follow‑up on incident reports to DHS and CHI.
  • Submit program reports on schedule to CHI, DHS, and oversight agencies.
  • Comply with funding contracts, partnership agreements, and applicable laws; complete corrective action plans as directed.
  • Attend required trainings and obtain certifications per OTDA and DHS rules.
  • Ensure tenants pay rent on time; develop rent‑based budget schedules; assist with arrears agreements.
  • Counsel tenants on housekeeping and neighbor behaviors.
  • Coordinate with Property Management for essential services and timely repairs/maintenance.
Qualifications
  • Bachelor’s degree in Social Work or Counseling preferred; 3+ years of experience in supportive housing or similar field can override the education requirement.
  • At least 2 years of experience in social work, human services, counseling, or case management.
  • Professional verbal and written communication skills.
  • Proficient in MS Word, Excel, Outlook, Internet; experience with CARES and AWARDS a plus.
  • Ability to develop and manage program budgets.
  • Physical capability to conduct unit visits (walk distances, climb stairs).
  • Strong reasoning ability; prioritize, make decisions and judgment calls.
  • Experience working with homeless or formerly homeless individuals and families.
  • Experience in needs assessment, crisis intervention, de‑escalation; behavioral management and motivational interviewing a plus.
  • Ability to manage demanding caseload with competing priorities.
  • Ability to work independently and as part of a team; leadership potential.
  • Highly organized, detail‑oriented; flexible; able to work well under pressure and meet deadlines.
