Schedule:

Monday–Friday, 9:00 AM – 5:00 PM

As a Case Worker, you will provide critical social and emotional support to older adults and their families. Working under the supervision of Master’s-level staff, you will conduct in‑home and in‑office assessments, develop personalized case plans, and advocate for the resources necessary to help seniors maintain their autonomy and well-being.

What You'll Be Doing

Responsible for the full spectrum of social services, you will ensure clients receive the clinical and practical support necessary to navigate the challenges of aging.

  • Assessment & Evaluation:
    Conduct in‑home and in‑office assessments using standardized instruments (e.g., PHQ‑9, NSI) to evaluate social and emotional needs.
  • Case Planning:
    Develop and implement written case plans, including individual/family counseling and guidance to resolve complex challenges.
  • Advocacy & Benefits:
    Screen for eligibility and provide advocacy for benefits such as Medicare, Medicaid, SNAP, and public assistance.
  • Service Coordination:
    Arrange for essential services including home care, transportation, Meals on Wheels, legal aid, and psychiatric examination.
  • Financial Protection:
    Evaluate clients’ capacity to manage their affairs; assist in securing financial management or guardianship to prevent exploitation.
  • Community Engagement:
    Lead caregiver support groups, conduct community education, and represent JASA at community conferences and meetings.
  • Administration:
    Maintain accurate case records and reports utilizing program‑specific software; inform supervisors of crisis situations immediately.
What You'll Need
  • Education:

    Graduate of an accredited college or university with a BSW, BA/BS, or equivalent degree.
  • Technical

    Skills:

    Computer literacy, including the ability to learn program‑specific software; strong record‑keeping and writing skills.
  • Communication:
    Ability to read, write, and speak English; knowledge of Spanish may be helpful.
  • Travel:
    Ability to travel to clients’ homes using mass transit and navigate physical barriers (stairs, etc.) in residences.
