Position Summary Queens Community House is seeking a participant-centered Case Manager to join the Queens Connect Career Services team. This role works directly with young adults ages 18–24 to identify barriers, develop individualized service plans, and support participants in achieving their educational, career, and personal goals.

The Case Manager will provide comprehensive support services, coordinate referrals, monitor participant progress, and help ensure successful outcomes throughout program participation and beyond.

Caseload:
Approximately 60 participants annually across multiple program cycles

Alumni Support:
Ongoing outreach and follow-up support for up to 12 months after program completion

Key Responsibilities Case Management Develop individualized participant service plans with clear short- and long-term goals

Conduct assessments to help participants identify strengths, interests, and career pathways

Provide ongoing coaching, case management, retention, and follow-up support

Connect participants to QCH wraparound services and external community resources

Maintain accurate and timely participant records, case notes, and service documentation

Collaboration & Participant Support Partner with internal teams to connect participants with Family Support Services and other resources

Collaborate with Job Developers to create internship, employment, and career opportunities

Monitor participant attendance, engagement, and overall progress toward goals

Co-facilitate workshops focused on job readiness, workplace success, soft skills, and personal development

Administrative Responsibilities Maintain timely and accurate documentation across required databases and tracking systems

Participate in team meetings, staff trainings, and professional development activities

Represent Queens Community House at community events, partner meetings, and stakeholder engagements

Support participant recruitment, intake, enrollment, and onboarding processes

Ensure compliance with program requirements, policies, and reporting standards

Qualifications

Bachelor's Degree preferred

High School Diploma or GED required with relevant professional experience1–3 years of experience working with out-of-school youth, unemployed young adults, or workforce development programs

Skills and Competencies Strong interpersonal, communication, and relationship-building skills

Excellent organizational and time-management abilities

Ability to motivate and support participants from diverse backgrounds

Proficiency in Microsoft Office Suite and data management systems, or the ability to learn quickly

Ability to manage a full-time caseload and adapt to changing priorities

Commitment to equity, inclusion, and participant-centered service delivery
