DescriptionConcern Housing is seeking a Service Coordinator to assist supportive tenants with successful community integration. The role uses a person‑centered, strength‑based approach to facilitate linkages with community resources, provide individualized counseling and support, and ensure appropriate documentation and compliance with program requirements. The Service Coordinator is an essential resource and advocate for special‑needs tenants.
Days/HoursTuesday – Friday; 12pm – 8pm & Saturday; 8am – 4pm
Responsibilities- Facilitate tenant linkage with community support services such as care coordination, outpatient behavioral and physical health services, dual recovery supports, vocational services, entitlements and financial assistance, and peer supports.
- Use a person‑centered, strength‑based approach to develop quarterly Support Plans, Functional Assessments, and Support Plan Reviews; meet with residents to develop skills and review progress; and maintain all required documentation (progress notes, discharge plans, etc.).
- Provide individual counseling and support services to promote skill development and tenant growth, including wellness self‑management, medication self‑sufficiency, housing readiness, and financial competency.
- Employ a harm‑reduction approach to improve health care outcomes and monitor progress toward the triple aim.
- Ensure availability of the full range of housing services, including maintenance issues.
- Attend weekly supervision and staff meetings.
- Perform other duties as assigned by the Program Director or Supervisor.
- Remain current with all agency‑required compliance documents and trainings.
Bachelor’s Degree in Psychology, Rehabilitation, Social Work, or another related mental health discipline, plus 2 years’ experience working in a similar setting and/or with people with mental illnesses. The candidate must have strong verbal and written communication skills and the ability to make sound judgments regarding client care.
