Location: New York
Breaking Ground in New York is seeking a Case Manager to support individuals experiencing homelessness. You will provide case management services to clients including those with mental illness and/or substance use disorders, assisting them in transitioning to permanent housing.
The role involves maintain a caseload of approximately 25 clients, conducting assessments, connecting clients to services, and ensuring compliance with agency standards. A Bachelor’s Degree or equivalent and relevant experience are required.
