Last year, we assisted over 764,000 visitors, served over 11.7 million meals, and accessed $10.7 million in public benefits resources for our guests. Through an array of programs that we offer, we look to establish long‑term stability for those we serve.
The Social Care Network Resource and Referral Case Manager will work with various database referral platforms (e.g., Public Health Solutions, Unite Us, Aunt Bertha, Food Bank TEN) to establish contact with public health and hospital systems and connect households to appropriate services within NYCP. Specific duties include monitoring referral systems, providing coordinated receipt of referrals, assessing needs, connecting to NYCP services, following up internally and externally, maintaining reporting tools, and conducting benefits screenings and direct assistance with public benefits applications (e.g., SNAP, TANF, Safety Net, Rental arrears).
The Help 365 Bilingual Mobile Case Manager reports to the Social Care Network Resource and Referral Manager at 8 East 109th, New York, NY 10029.
Schedule:
Monday‑Friday, 9 a.m. – 5 p.m. (1‑hour lunch). This is a full‑time, non‑exempt position (35 hours per week).
- Travel throughout New York City to assigned partner sites, community locations, and outreach events to provide benefits enrollment and social care services.
- Develop and maintain relationships with community partners and identify opportunities for expanded benefits enrollment services.
- Manage assigned caseloads from partner sites and social care referral platforms (e.g., Unite Us, Mount Sinai clinics, Mobile Partners).
- Provide direct enhanced services to participants in accordance with Whole
YouNYC program guidelines established by Public Health Solutions and Unite Us. - Conduct outreach and follow‑up with referred individuals and families to connect them with food assistance, public benefits, healthcare coverage, and other supportive services.
- Accurately document all services provided using contracted service notes to ensure compliance with program requirements; ensure timely and proper case closure upon successful delivery of services.
- Screen participants for eligibility, assist with applications and recertifications for public benefits programs (SNAP, Cash Assistance/TANF, One Shot Deal, SSI/SSD, Medicaid, Fair Fares, and other resources).
- Facilitate formal mediation between applicants and HRA as needed to secure benefits.
- Confirm receipt and amounts of awarded benefits upon application completion.
- Provide participants with vetted information about local community resources.
- Elevate complex or high‑priority cases to supervisors as needed for guidance and support.
- Other responsibilities as assigned.
- An Associate’s degree in Social Work or Human Services.
- A minimum of two years of relevant work experience.
- Full professional proficiency in English and Spanish.
- Ability to travel between multiple job locations.

- Knowledge of Access HRA and HRA applications.
- Experience with public benefits, entitlements, and associated systems (e.g., SNAP, TANF, One Shot Deal, ERAP).
- Proficiency with Microsoft Office Suite and Google Drive.
- Professional data‑entry and spreadsheet skills, and ability to learn new software quickly.
- Professional written and verbal communication skills.
- Ability to multi‑task and work in a highly trafficked area.
