Youth Advocate - Case Manager for At-Risk Youth

Job Description

Annual Non-Exempt Salary: $37,849.50

Job Duties:

The Case Manager will serve youth with disabilities who may be at risk due to any of the following: behavioral issues, a lack of community-based support services, lack of achievement in school or lack of social skills.

The successful candidate will be expected to:

• Conduct a youth risk assessment to determine their potential for crisis.
• Complete a Support Service Plan to address identified needs.
• Maintain regular contact with participants and their families to monitor progress, including visits at mutually agreed upon locations (including the home).
• Maintain and/or update case files.
• Provide information and referral services.

Knowledge, Skills & Abilities:

• Experience working with youth and/or individuals with disabilities, either professionally or personally.
• Strong advocacy, networking, and organizational skills.
• Computer skills, including Outlook and Microsoft programs.
• Knowledge of community-based services.
• Excellent customer service skills.

Education:

• Associates Degree is preferred.
• High School Diploma or equivalent is required.
• Experience may be considered in lieu of higher education.
