Case Manager – Bronx, NY | $25 - $26/hr | Temp to Perm
We’re hiring a Case Manager to support individuals and families—especially those with SPMI—to achieve stability and independence.
Location: Bronx, NY
Pay: $25 - $26/hour
Schedule: Full-time (35 hrs/week), Temp-to-Perm
What you’ll do- Assess clients and develop service plans
- Connect clients to housing, healthcare and community resources
- Provide ongoing support, advocacy and documentation
- Bachelor’s degree
- Experience with SPMI or vulnerable populations
- Strong communication and organizational skills
