Location: New York
Comunilife Inc is seeking experienced Case Managers for its OMH Adult Homes program in Queens. The Case Manager will provide case management services for clients, ensuring appropriate assessments and service plans while conducting home visits and fieldwork.
The ideal candidate must possess a Bachelor's Degree and have 1-2 years of relevant fieldwork experience, particularly in mental health or substance abuse services. Familiarity with Scattered Sites Programs and bilingual capabilities in Spanish are preferred.
