OverviewThe Housing Works Inc. Reentry Services Program is a collaboration between Housing Works Inc. and The Mayor’s Office of Criminal Justice (MOCJ) to provide comprehensive, holistic reentry services to individuals with criminal justice involvement reintegrating into the five borough communities. Reporting to the Community Program Manager, the Reentry Case Manager will be responsible for providing community-based case management services to clients who meet the program eligibility criteria.
These case management services include conducting psychosocial assessments, co-developing treatment plans, facilitating therapeutic soft skills groups, connecting to health and wellness services, assisting with benefits and entitlements enrollment, and ongoing outreach for the duration of program engagement. The Reentry Case Manager will be solution-focused, experienced in providing clients with wraparound case management services, and knowledgeable of NYC resources and benefits available for justice-involved clients.
- Provides supportive case management to MOCJ Transition and Community-Based Program clients.
- Conducts intake assessments and develops action plans based on the needs identified in the assessment.
- Refers clients for health care services through Housing Works and with external referral partners as needed.
- Conducts client outreach weekly, including phone calls, text alerts, appointment reminders, letters, and home visits.
- Responsible for obtaining weekly client compliance reports from external referral or continuing education services.
- Maintains essential data for reporting and program evaluation in eICare, including program enrollment, progress notes, care plans, referral activity, and updating contact information.
- Builds and maintains resource lists for external services and emergency services (housing, domestic violence, food pantry, etc.).
- Facilitates evidence-based groups weekly.
- Participates in weekly supervision sessions with the Community Program Manager.
- Participates in monthly all-staff program meetings and bi‑weekly team meetings.
- Promotes Housing Works’ core values in day‑to‑day activities.
- Performs other tasks as assigned by supervisor.
- Provides culturally competent services in accordance with Housing Works policies and practices.
- Promotes Housing Works core values consistently.
- Has ability to work with diverse clients with varying levels of service needs.
- Works collaboratively with other team members within the Justice Initiatives Department and cross‑agency.
- Attends and participates in staff meetings and trainings as scheduled.
- Responsive to emails, texts, and phone calls.
- Adaptable and capable of successfully upholding job functions in a full remote or hybrid workforce environment.
- Adheres to remote workforce policies, procedures, and protocols.
- Bachelor’s Degree required in social services, psychology, social work, or other related field.
- Minimum 1 year experience providing case management or direct client service.
- Experience with criminal justice involved population, as well as substance use and mental health issues.
- Strong written documentation and communication skills.
- Good problem‑solving skills and ability to work autonomously.
- Microsoft Office Suite, particularly Excel, required.
- Preferably bilingual in Spanish.
- Excellent written and oral communication skills.
- Excellent or proficient technical skills to perform tasks related to Microsoft Office, email, and case management systems.
- Ability to work independently and collaboratively with the team.
