Volunteers of America–Greater New York (VOA–Greater New York) is a leading anti‑poverty organization serving neighbors in need across New York City, Northern New Jersey, and Westchester. Through 70+ programs, we support over 35,000 adults and children each year, including individuals experiencing homelessness, survivors of domestic violence, youth aging out of foster care, veterans, seniors, and children with developmental delays.

Case Manager/Client Care Coordinator

The Case Manager/Client Care Coordinator is a key provider of services within Volunteers of America’s Case Management Model. The Case Manager works with the client to assess their strengths and needs, plan a course of action, link the client to services, counsel the client, and monitor progress. The role ensures that clients receive appropriate, competent and professional information, advice, referrals and advocacy services to achieve successful outcomes.

The position may include supervision of direct care staff.

Victory I is a facility that provides comprehensive services and safe, anonymous housing for individuals and their children experiencing domestic violence.

Minimum Qualifications
  • Associate’s degree or 60 college credits in Human Services field or High School Diploma or GED with 2 years of related experience.
  • Demonstrated ability in verbal and written communication, including basic computer skills.

Candidates with related experience with domestic violence are preferred.

Principal Responsibilities
  • Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
  • Develop with the client a mutually agreed‑upon individual service plan reflecting assessment findings and goals; update the plan with the client as required.
  • Meet regularly with the client to assist them in obtaining needed services/benefits to achieve their individual service plan goals and provide supportive counseling to strengthen their ability to make appropriate life decisions.
  • Document case management activities in accordance with agency and funder guidelines and procedures; collect service data and participate in evaluation and quality assurance activities.
  • Provide aftercare to assigned clients who have been placed or discharged from the facility.
