  • Pay or shift range: $23 USD to $26 USD. The estimated range is the budgeted amount for this position. Final offers are based on various factors including skill set, experience, location, qualifications and other job-related reasons.

Compensation Range: $23/hr - $26/hr commensurate with experience.

Benefits:
We have three comprehensive healthcare plans to choose from. Housing Works covers most of each plan; you pay a portion, based on your salary. Staff accrues PTO immediately for up to 30 days earned in the first year. An educational benefit is available for tuition loan reimbursement, tuition costs, and textbooks.

Overview:
The Housing Works Emergency & Transitional Housing Program (ETH) provides time‑limited transitional housing and support services to individuals re‑entering the community following periods of incarceration or other justice system involvement. Program participants engage in a variety of onsite services, including case management, vocational training, housing advocacy, and mental health support that address barriers to accessing housing and help build skills for stable community living.

Reporting to the Assistant Director, the Intake Coordinator plays an integral role managing program referrals and coordinating admission. Activities include managing the program wait‑list, collaborating with internal and external referral partners, screening and reviewing referrals for eligibility and site placement, data entry and record keeping, and managing initial admission and enrollment. This position suits professionals with social services and data entry experience seeking to further develop skills in the reentry and housing sector.

Responsibilities

Primary Responsibilities
  • Closely review and monitor program census and submit daily vacancy reports.
  • Review and screen referrals for eligibility and appropriate program site placement.
  • Assist with gathering additional supporting documentation or medical records when necessary.
  • Coordinate with interdisciplinary care teams for further evaluation and screening of referrals to determine level of care needs.
  • Manage the program wait‑list using the electronic record system.
  • Coordinate with referral partners or candidates for admission when there is a vacancy.
  • Facilitate recurring meetings with key stakeholders and referral agencies to provide status updates on submitted referrals.
  • Complete daily, weekly, monthly and quarterly reports timely and accurately.
  • Monitor and report referral and placement outcomes.
  • Ensure data entry is accurate and timely.
  • Create new charts in the electronic medical records system for all newly arriving clients.
  • Promote Housing Works’ core values in day‑to‑day activities.
  • Perform other tasks as assigned by supervisor.
Secondary Responsibilities
  • Participate in agency advocacy events.
  • Work collaboratively across teams and divisions.
  • Actively participate in supervision and professional development trainings.
Other Functions
  • Respond promptly to emails, texts, and phone calls.
Requirements
  • Bachelor’s Degree in social services, psychology, social work, or a related field, or an Associate’s Degree with 2+ years of social services or data entry experience.
  • Experience working in a healthcare or non‑profit setting.
  • Strong written documentation and communication skills.
  • Good problem‑solving skills and ability to work autonomously.
  • Proficiency in Microsoft Office Suite, particularly Excel.
  • Preferred bilingualism in Spanish.
  • Excellent technical skills for Microsoft Office, email, and case management systems.
  • Ability to work independently and collaboratively with the team.
  • Dependability and reliability to arrive and depart promptly for scheduled shift.
  • Ability to travel frequently within the boroughs via public transportation.
