Compensation Range: $25/hr - $28/hr commensurate with experience

OverviewThe Housing Works Inc. Alternatives to Incarceration (ATI) Program is a collaboration between Housing Works and the Mayor’s Office of Criminal Justice (MOCJ) to provide court‑mandated, risk‑need‑responsivity services to jail‑bound individuals 18 years of age and older who are involved in Manhattan criminal and supreme court systems. Under the supervision of the Clinical Coordinator, the Case Manager provides comprehensive, holistic case management support services to clients enrolled and engaging in ATI program services.
These support services include referrals to additional medical and behavioral health services, treatment planning and treatment‑plan reviews, benefits enrollment, connection to external community resources, and program compliance support. The Case Manager will be knowledgeable of court systems and operations and criminal justice risk‑need‑responsivity interventions, and will assist clients with mental health, substance use, and behavioral health related issues and the unique needs of the criminal‑justice‑involved population.
Primary:
- Provide supportive case management to Housing Works ATI Program clients while engaged in program services
- Refer and connect clients to wrap‑around services such as healthcare, vocational training, continuing education, etc.
- Assist clients with obtaining IDs and vital documents and benefits enrollment
- Engage in care coordination activities with clients and the interdisciplinary care team
- Support clients with adherence and compliance with court mandates
- Conduct client outreach weekly, including phone calls, text alerts, appointment reminders, letters, and home visits
- Obtain weekly client compliance reports from external referral or continuing‑education services
- Maintain essential data for reporting and program evaluation in eICare, including program enrollment, progress notes, care plans, referral activity, and updating contact information
- Build and maintain resource lists for external services and emergency services (e.g. housing, domestic violence, food pantry, etc.)
- Facilitate evidence‑based groups
- Participate in weekly supervision sessions with the Clinical Coordinator
- Participate in monthly all‑staff program meetings and bi‑weekly team meetings
- Perform other tasks as assigned by supervisor
- Display Housing Works core values in daily activities
Secondary:
- Work collaboratively with multi‑disciplinary OASAS and Reentry teams
- Provide culturally competent services in accordance with Housing Works policies and practices
Other functions that may be required:
- Attend and participate in staff meetings and trainings as scheduled
- Respond to emails, texts and phone calls promptly
- Participate in advocacy events and calls to action

- Bachelor’s Degree required
- 1‑3 years of relevant experience working in human services with individuals involved in the criminal legal system with mental health, substance use or behavioral health needs
- Skilled in delivery of motivational interviewing, cognitive behavioral techniques, and risk‑need‑responsivity interventions
- Preferably bilingual in Spanish
- Excellent written and oral communication skills
- Excellent or proficient technical skills to perform tasks related to Microsoft Office, email and case management systems
- Ability to work both independently and collaboratively with the team
- Dependability and reliability to arrive and depart timely for scheduled shift
- Cultural sensitivity and trauma‑informed approach to care
- Comfort working within the courts or secured DOC assigned areas
