Location: New York
6AM City, LLC is seeking a Case Manager/Service Coordinator for its Safe Options Support (SOS) Team based in New York City. In this role, you will work closely with a multi-disciplinary team to provide comprehensive care to street homeless individuals.
The Case Manager will facilitate community outreach, coordinate services, and ensure participants maintain stability in their housing situations. A Bachelor’s degree and previous case management experience are required, with a preference for bilingual candidates.
Generous benefits package included alongside a competitive salary of $60,000.
