Job Title: Transitional Care Case Planner
Program: Congregate Care
Salary: $57,000 - $60,000 (depends on educational achievements and experience)
General Responsibilities:
Catholic Guardian Services (CGS) is seeking an experienced and dedicated Transitional Case Planner to join our foster care residential program. Our program provides vital services and support to youth facing psychological, behavioral, and/or substance use challenges.
The Transitional Case Planner will be responsible for delivering comprehensive case planning and case management services to youth on long-term (60+ days) suspended payment status, including those who are AWOC (Absent Without Consent), incarcerated, hospitalized, or on extended visitation status.
Required Skills:
- Bachelor Degree in human services field
- Two plus years of child welfare experience
- Ability to travel to residential facilities, agency offices, foster homes and parent/discharge resource homes.
Desired
Skills:
- Master’s degree in Social Work/related field
- Ability to work a flexible schedule, including evenings and weekends, as necessary, and be
available for on-call duty for crisis interventions and consultation. - NYS Driver’s license preferred.
