Case Planner II
The Case Planner II plays a vital role in helping children achieve safe, permanent, and nurturing homes through reunification, adoption, KinGAP, or preparation for independent living. This position is ideal for a compassionate, skilled professional who believes in family engagement, strengths-based practice, and timely permanency.
The Case Planner will partner with children, families, caregivers, and community providers to ensure every child's safety, well-being, and long-term stability.
Responsibilities- Assess safety, risk, protective, and promotive factors and collaborate with families to develop and implement safety plans.
- Engage children and families in identifying strengths, resources, and supports, and actively involve them in service planning and decision-making.
- Partner closely with caregivers and foster parents to support treatment and promote positive outcomes for children in care.
- Collaborate with parent advocates to support parents in addressing challenges and working toward reunification when appropriate.
- Engage families in concurrent planning from the beginning of child welfare involvement.
- Identify, develop, and support family, kinship, and non-kinship resources to achieve permanency goals.
- Develop and monitor visitation plans for youth and families, including sibling connections.
- Coordinate and facilitate family, sibling, and permanency meetings, supervising visits as required by the service plan.
- Support youth in preparing for independent living, including monthly IL contacts and documentation.
- Coordinate services with caregivers, schools, medical providers, mental health professionals, and community partners.
- Maintain regular collateral contact with interdisciplinary teams and service providers to assess progress and effectiveness.
- Collaborate with Home finding to identify, develop, and certify viable discharge resources.
- Work with internal JCCA divisions to ensure children are placed in the least restrictive, most appropriate level of care.
- Partner with youth skills coaches to help youth develop age-appropriate life skills.
- Ensure compliance with all Agency, City, State, and Federal requirements related to documentation, visitation, and client contact.
- Prepare and maintain accurate, timely case documentation, including: progress notes, FASPs and Psycho-socials, Plan Amendments, Court reports and testimony, Adoption summaries and home studies, CCS
21+ applications. - Ensure preparedness and participation in all legal proceedings, including Family and Criminal Court.
- Complete Special/Exceptional Rate Requests and reauthorizations as needed.
- Provide crisis intervention as needed, including support during hospitalizations and discharge planning.
- Serve as a mentor to newer staff and support onboarding efforts within the department.
- Implement evidence-based models with children, caregivers, parents, and discharge resources.
- Attend team meetings, supervision, consultations, and required trainings, including evidence-based model training.
- Provide outreach, advocacy, and family support services as needed.
- Perform additional duties as assigned to support program and client needs.
- Master's degree required (MSW preferred).
- Ability to engage clients and build strong, professional helping relationships.
- Knowledge of community resources and ability to navigate complex service systems for multi-problem families.
- Experience working with substance-use-affected populations preferred.
- Strong computer skills, including typing and documentation proficiency.
- Excellent interpersonal, communication, and organizational skills.
- Bilingual English/Spanish preferred.
- Valid driver's license required.
Hourly Rate Range: $32.05 to $33.34
