Direct Support Professional – Community Connections
Job Path supports autistic people and people with developmental disabilities as they make choices about their lives and play valued roles in their communities.
Responsibilities- Provide individualized support to connect people to their communities and help them become contributing community members.
- Work with families and community members to develop connections with community organizations.
- Assist with volunteer jobs, classes, social events and other community activities.
- Help individuals meet personal and professional goals.
- Build self‑esteem by doing things together rather than doing for them.
- Provide opportunities for new experiences, especially indoor activities.
- Encourage individuals to make choices and decisions in all areas of their lives and help them learn how to decide.
- Support people in social situations to become comfortable and confident through appropriate interactions and by building positive relationships.
- Ensure clear, concise and honest communication with supervisor.
- Offer support, respect and feedback to other team members.
- Attend staff meetings and contribute professionally.
- Share and contribute ideas, knowledge and questions.
- Be aware of and competent in dealing with medical needs such as seizures, health concerns, allergies, etc.
- Provide physical support and assistance with ADLs in a dignifying and respectful manner.
- Complete case notes, monthly summary notes, and timesheets by assigned deadlines.
- Follow up with required documentation as detailed by supervisor.
- High school diploma or GED, written and spoken English.
- One year of prior experience working with people with disabilities (residential experience with ADL support a plus).
- Enjoy active, people‑oriented work and are sensitive to needs and preferences of people with disabilities (includes applicants who are autistic or have disabilities).
- Basic computer skills; ability to respond to e‑mails, submit electronic timesheets, and complete casenotes through an electronic health record system.
- Strong written and verbal communication skills.
Monday to Friday, 1 pm to 6 pm.
LocationCrown Heights, Brooklyn.
Rate of Pay$19.44 per hour.
