Position Summary
The Socio-therapist ensures that all therapeutic youth have a behavior plan that addresses individualized behavioral strengths and challenges pertaining to the specific youth. The Socio-therapist will support the clinical team in implementing and monitoring the plan of care for the youth on their caseload.
Essential Responsibilities- Creates, in partnership with families, plans which address safety, behavioral, psychological and permanency needs.
- Monitors safety, well-being, and permanency progress by conducting regular casework contacts and providing the necessary services to meet the needs of the children and families.
- Complete timely documentation of all reports and casework activities including FASPS, Psychosocials, Adoption Home studies, Kinship Home studies, court reports.
- Actively participates in Family Court Hearings, Family Team Conferences, and other meetings to advance permanency for children and families.
- Provide crisis intervention as needed.
- Implement evidence-based models with foster parents, parents, discharge resources and youth.
- Coordinate care with other agency and community-based programs such as Behavioral Health & Wellness and Health Homes.
- Provide family planning counseling to youth 12 years and older and document discussion with foster parents.
- Work to identify family resources and support for youth through Family Finding evidence-based practice.
Hybrid work schedule – remote days available
Qualifications- Bachelor's Degree required – preferably BSW.
- Experience supporting families and adolescents with social services. Must have a commitment to work from a strength-based, youth development, and trauma-informed perspective.
- Ability to work with a diverse population of clients and team members.
- Must be highly organized with ability to meet program deliverables and deadlines.
- Must have excellent verbal and written communication skills.
- Ability to work flexible hours – some evenings as needed to meet client needs.
- Ability to use public transportation to travel throughout New York City or Agency vehicle when available.
