Position: Client Care Coordinator/ Social Worker (Cauldwell House)
Location: New York

Primary Function/Purpose

The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. The coordinator will enhance existing services to include a thorough assessment of clients’ long‑term goals at intake and collaborate with case‑management staff to set achievable goals while establishing linkages within the community to ensure the client’s successful progress towards independent living.

This individual will work with clients at our Cauldwell location in the Bronx, NY.

Minimum Qualifications
  • Must be a Licensed Master Social Worker (LMSW) in New York upon hire or obtain licensure within three months and have a master’s degree in social work from an accredited program.
  • At least one year of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services is strongly preferred.
  • Knowledge and experience working with diverse cultures and ethnicities.
  • Expertise in strengths‑based, solution‑focused, and family‑centered practice.
  • Experience facilitating groups for adults.
  • Proficiency in Spanish and/or French is a plus.
  • Excellent written and verbal communication skills.
  • Excellent interpersonal skills and computer literacy in Microsoft Office.
Primary Responsibilities
  • Complete a comprehensive bio‑psychosocial assessment with each family to understand strengths and service needs.
  • Provide clear written reports that capture assessment findings and recommendations.
  • Assist families that are homeless as they navigate multiple systems and cope with the stressors and anxiety induced by homelessness.
  • Provide outreach services, crisis intervention, risk assessment, safety planning, and psychoeducation for families.
  • Prioritize and conduct unit visits to high‑risk families in shelter to coordinate safety planning.
  • Prepare children and parents to accept services.
  • Collaborate with ACS and/or prevention agencies, participate in conferences, and advocate on behalf of the family.
  • Make appropriate referrals and facilitate linkages between mental health providers, government agencies, and other community‑based services.
  • Serve as agency or program liaison between government agencies, community organizations, and groups.
  • Improve access to mental health services for families in shelter by facilitating in‑person or telehealth services.
  • Enhance delivery and coordination of mental health and related services.
  • Work effectively as part of a multi‑disciplinary team to enhance engagement and ensure quality service.
  • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well‑being.
  • Deliver enhanced mental health services: conduct biopsychosocial assessment, provide on‑site counseling and brief intervention services, coordinate clinical treatment referrals, and facilitate linkage to care and follow‑up.
  • Promote in‑person or telehealth usage through H+H Express Care and other telehealth services via hospitals and mental health providers chosen by the client.
  • Utilize DHS Referral Basics to match the need assessed through the BPS, assist clients in scheduling, provide reminders, coordinate transportation, facilitate access to teletherapy, and provide a warm hand‑off when possible.
  • Follow up on referrals: confirm attendance, assess outcomes, address concerns or barriers, and reschedule if necessary.
  • Employ motivational interviewing when a client is reluctant to participate or be linked to treatment.
  • Provide therapeutic groups (music, art, general support groups for caregivers or children) and facilitate group work to address family issues and enhance life skills.
  • Participate in family meetings with case management and housing staff to discuss planning efforts and progress.
  • Assist clients with completing supportive housing applications and accompany residents to interviews when necessary.
  • Upon linkage to permanent housing, complete exit summaries and provide referrals and supportive services for the new community.
