Position Objective

Provides comprehensive intake, assessment, and service planning for youth ages 18–24 referred to the PACE program. Supports young adults in identifying career pathways, educational opportunities, and setting realistic, individualized goals. Guides youth through the career decision‑making process by helping them explore their strengths, interests, and prior experiences. Ensures engagement by assigning activities and opportunities that are age‑appropriate, developmentally supportive, and aligned with each youth’s career and educational aspirations.

Functions
  • Works with participants to develop a comprehensive service plan based upon client intake, assessment results, and thorough understanding of the individual’s interests, strengths, employment and training history, preferences, and needs. This plan will be completed within 30 calendar days of client enrollment to PACE.
  • Conducts a one‑on‑one interview to collect information about the participant’s personal work history, barriers to employment, criminal history, personal/family background, and living situations. Identifies barriers and issues that may result in participant being returned to HRA for further evaluation.
  • Conducts intake and internal assessment to determine client’s assigned Service Tier (Tier 1, Tier 2 or Tier
    3).
  • Maintains and executes documentation and workflow through computer‑based programs and technology that draws on basic software/computer skill set including case notes for each client interaction, client intake signature, scanning relevant documents to client file such as signed EP/ISP, education credentials and resume.
  • Assists with the assignment of program participants into Career Education Services, Alternative Engagement Opportunities, Internship and/or Community Service, Bridge programming as appropriate, Job Placement or other HRA programs/services. Meets monthly goals identified in this area. Utilizes program flyers, information sessions, link system and informational emails for this purpose.
  • Collaborates with Community Partner Liaison to identify slot availability of trainings, internships, community service and educational opportunities for client referrals to Alternative Engagement and concurrent assignments and other HRA programs/services.
  • Conducts re‑assessments on clients returned to PACE. Provides case management, counseling, assessment(s) and assists in the re‑assignment to programming that might be a better fit. For clients disengaged for more than 3 months, provides counseling and support to determine the reasoning for not participating.
  • Taking the results of the TABE or SABE and other types of assessment instruments used in PACE, determines the educational, skills level, and vocational interest of the participant. Completes HRA’s online employment plan.
  • Consistently manages caseload by reviewing client program start dates, attendance and other issues to best determine next steps for clients, including referrals to external programs, timely FTCs or other necessary actions.
  • Collaborates with other PACE staff to provide holistic services to all clients during orientation, workshop and service coordination meetings. Collaborates with Post‑Assessment Coordinator to maintain follow‑up and accurate information regarding client progress at referral engagements for at least 1 year following assessment completion.
  • Collaborates with Post‑Assessment Coordinator to generate client success stories every six weeks. The success stories should detail how the assessment process assisted client with determining their goals, and what progress they have made toward those goals.
