Job Overview – Youth Healthcare Support Specialist (Bilingual Spanish)

Compensation: $28/hour
Location: New York, NY
Schedule: Monday to Friday (Hybrid)

TAG Med Staffing is hiring a Youth Healthcare Support Specialist (Bilingual Spanish) in New York, NY for our client, supporting adolescents and families within a community-based behavioral health environment. This role focuses on youth advocacy, behavioral support, crisis intervention, case coordination, and care planning while partnering closely with families, care teams, and community resources. The ideal candidate has experience working with youth populations, strong relationship-building skills, and the ability to support families through a strength-based and trauma-informed approach.

Responsibilities
  • Youth & Family Support: Conduct weekly in-person home visits with youth and foster families to support behavioral goals, life skills development, and daily care needs.
  • Case Management: Maintain accurate case records, progress notes, assessments, behavior plans, and documentation related to client services and case activity.
  • Crisis Intervention: Provide direct crisis support and collaborate with internal teams, foster parents, and external stakeholders to address urgent client needs.
  • Behavioral Planning: Partner with youth, foster parents, and case workers to develop, implement, and monitor individualized behavioral goals and treatment plans.
  • Community Coordination: Connect youth and families with educational, recreational, employment, and community-based resources to support long-term development and stability.
  • Trauma-Informed Care: Apply strength-based, trauma-informed, and youth development practices to support positive outcomes and a safe therapeutic environment.
Qualifications
  • Education: Bachelor’s degree in Social Work, Psychology, Human Services, or a related field is required.
  • Language

    Skills:

    Bilingual English and Spanish language skills are required.
  • Experience: 1+ years of experience working with adolescents, youth services, foster care, behavioral health, or family support programs is required.
  • Industry Knowledge: Understanding of trauma-informed care, youth development practices, behavioral interventions, and foster care environments is required.
  • Technical

    Skills:

    Proficiency with case management documentation, Microsoft Office Suite, and written reporting is required.
  • Skills & Attributes: Strong communication skills, relationship-building abilities, crisis management skills, organizational abilities, and the ability to engage youth and families effectively are required.
