Job Overview – Care Coordinator (Case Management)
TAG Med Staffing is hiring a Care Coordinator in Brooklyn, NY for our client, supporting care management, case coordination, and community-based services for individuals with complex medical, behavioral health, and social service needs. This role will coordinate care plans, connect clients with healthcare and community resources, and advocate for clients to improve health outcomes and long-term stability. The ideal candidate will bring strong case management experience, excellent communication skills, and a passion for serving diverse populations.

ScheduleFull-Time (35 hours per week)
Responsibilities as the Care Coordinator (Case Management)- Client Intake & Assessment: Conduct intakes, eligibility reviews, and reassessments to determine client needs and services.
- Case Management: Manage a caseload of clients, develop service plans, and monitor progress toward goals.
- Client Advocacy & Support: Serve as a liaison with healthcare, housing, legal, and social service providers while escorting clients to essential appointments as needed.
- Progress Monitoring & Documentation: Track client outcomes through visits and calls, maintain accurate service plans and progress notes, and prepare required reports.
- Community Engagement: Facilitate support groups, coordinate events, and assist with outreach, benefits applications, and program promotion.
- Education: Associate's or Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
- Experience: 2+ years in case management, social services, or community health, with experience supporting HIV/AIDS populations strongly preferred.
- Technical
Skills:
Proficiency in Microsoft Office Suite and experience with electronic case management or client database systems. - Industry Knowledge: Understanding of HIV/AIDS, Medicaid, entitlements, and community-based health and social services.
- Skills & Attributes: Strong interpersonal, communication, and problem-solving skills with the ability to advocate for clients and build trust.
- Language
Skills:
Bilingual in English and Spanish or Haitian Creole preferred. - Additional Requirements: Ability to travel locally to client homes and appointments as needed.
