Location: New York
We are seeking a licensed Social Worker to serve as Team Leader for an Assertive Community Treatment (ACT) program in Manhattan. This role provides both clinical leadership and hands‑on support for a multidisciplinary team serving adults with serious and persistent mental illness who require intensive, community‑based care.
The annual base salary range is $70,000 to $76,000. Actual compensation offered to the successful candidate may vary from the posted hiring range based upon geographic location, work experience, education, and/or skill level, among other factors. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities- Provide clinical oversight and direct case management services to a small caseload
- Conduct client assessments and develop individualized treatment goals
- Support client access to psychiatric care, medication management, entitlements, and community services
- Coordinate care with hospitals, residential facilities, families, and external providers
- Accompany clients to appointments and conduct visits in the community as needed
- Maintain accurate and timely client documentation in accordance with program and funder requirements
- Participate in rotating on‑call coverage outside of regular business hours
- Provide ongoing clinical supervision to staff and interns, including case review and skill development
- Support hiring, onboarding, and training compliance for ACT team staff
- Monitor staff documentation, productivity, and reporting across required systems
- Assist with staff scheduling, policy development, and daily program operations
- Serve as acting Program Director when needed
- Participate actively in ACT team meetings, case conferences, and trainings
- Work within a shared caseload model to support client stability and engagement
- Provide coverage for clinical and administrative functions as needed
- Active LMSW required
- Experience working with adults with serious and persistent mental illness
- Prior ACT Team experience strongly preferred
- Proficient with Microsoft Word and Outlook
- Experience with or willingness to learn AWARDS, CAIRS, NYCMED, and other funder‑required systems
- Strong written and verbal communication skills
- Highly organized with strong attention to detail
- Comfortable working independently in the field and in office‑based settings
- Combination of field‑based and office‑based work
- Regular community travel, including use of public transportation
