Location: New York
Project Renewal Inc. is hiring a Case Manager to support clients across multiple shelters in the Bronx, Brooklyn, and Manhattan.
You will conduct intake assessments, develop Independent Living Plans, and connect clients to housing and benefits, while coordinating care with clinicians and maintaining case records. Bilingual capabilities are a plus and a strong commitment to serving homeless populations is required.
