Location: New York
JASA is seeking a dedicated Case Manager to support older adults at the Roy Reuther and Rockaway Park Older Adult Centers. You will provide information, referrals, advocacy, and direct case assistance to promote independence and well-being.
You will screen for benefit eligibility, assist with applications, and connect clients to transportation, home care, meals, Medicare/Medicaid, and housing resources. Evening/weekend availability may be required.
