Covenant House New York invites applications for a Case Manager to provide direct, supportive services to youth in our short-term emergency Youth Shelter. You will work within an integrated, multidisciplinary team to promote housing stability, personal growth, and service-plan goals.
You will collaborate with youth to assess strengths and challenges, develop ISP plans, and follow up on progress, while maintaining confidential records and adhering to regulatory requirements.
