Position Summary:
We have an exciting opportunity to join our team as a Family Assessment Worker
- Brooklyn
- Spanish Required.

In this role, the successful candidate The Family Resource Specialist/Family Support Specialist is responsible for conducting outreach and screening to identify pregnant or newly parenting caregivers eligible for home visiting services. This position carries a caseload of families with the goal to support positive parent child bonding, promote child and family health and development, and enhance family resilience. Reporting to the Program Supervisor these home-visiting services are delivered within the context of a health care system in collaboration with internal and external stakeholders.

Job Responsibilities:

  • Conduct outreach and screening to identify families eligible for home visiting services.
  • Initiate and maintain regular contact with families, primarily via visiting them in their homes.
  • Documenting initial family assessments and using assessment tools as indicated.
  • Interviewing parents, preferably prenatally and/or immediately after the birth of their child.
  • Reviewing Hospital or Clinic records.
  • Timely reporting and case documentation in an electronic database management system
  • Assisting in making and attending health and human service appointments, including activities related to employment and educational goals.
  • Assisting parents to improve their skills to optimize the home environment for the child's well-being and safety.
  • Assisting to strengthen parent-child relationships.
  • Providing prenatal and other health education.
  • Establishing a trusting relationship with families.
  • Participate in community events to promote Healthy Families services.
  • Establish partnerships with internal and external agencies that work with families.
  • Performs other related duties as assigned.

Minimum Qualifications:
To qualify you must have a High School diploma/GED with experience working with, or assisting at risk children and families in a community setting.
Bilingual Spanish.

Preferred Qualifications:
Knowledge of infant and child development
Infant mental health endorsement preferred (if available in the state)
Ability to observe and report accurately on the functioning of individuals and families
Ability to establish trusting relationships and work effectively with mothers, fathers and extended family
Successful parenting and/or child care experience
Ability to handle stressful situations
Emotionally mature and capable of exercising judgement
Respect for and sensitivity to the needs and rights of others, including those with differing cultural, racial, sexual, or ethnic identities.

Qualified candidates must be able to effectively communicate with all levels of the organization.
