Location: New York

We’re looking for a Social Care Navigator with strong ties to Brooklyn or Queens communities to help Medicaid members access food, housing, transportation, and other essential supports. This role is ideal for someone who knows the neighborhoods, understands local resources, and is trusted by the community.

What You’ll Do
  • Conduct outreach and engage Medicaid members living in Brooklyn and Queens.
  • Assess health‑related social needs and connect participants to local resources.
  • Build relationships with community‑based organizations and service providers.
  • Use a referral and documentation platform to track services and follow‑up.
  • Share community insights to improve how services are delivered.
What We’re Looking For
  • Direct experience working in Brooklyn and/or Queens communities.
  • Background in care navigation, outreach, intake, case management, or community-based human services.
  • Strong knowledge of neighborhood resources, housing supports, and social service systems.
  • Ability to build trust with participants using culturally responsive, strengths-based approaches.
  • Comfortable working independently while collaborating with a team.
  • Lived experience and long-standing community involvement are highly valued.

No formal education requirement. Relevant community and service experience is what matters most.
