Job Title:Case Planner
Program:Enhanced Family Foster Care
Location: Harlem NY

General Responsibilities

Catholic Guardian Services is looking for a dedicated and experienced Case Planner for our Enhanced Family Foster Care program. The Case Planner supports the birth and foster parents’ perseverance with children through challenging periods of extreme and troubling behavior and models appropriate responses and teaching strategies. Guiding adolescent birth parents and foster parents through a proactive process of assessment, goal setting, and treatment planning to address the children’s current emotional/behavioral problems, accomplishes this task.

Requirements
  • BA/BSW,
  • 1-2 years child welfare related experience,
  • Prior experience working with adolescents and a passionate commitment to permanency for adolescents
  • Ability to work a flexible schedule, including periodic evenings and weekends, ability to travel to community settings throughout NYC.
Preferred (but not required)
  • Foster Care experience
  • Drivers license
  • MSW
