We are seeking a compassionate, organized, and self-motivated Social Worker / Geriatric Care Manager to join our growing team. This is an excellent opportunity for a social worker who is passionate about working with older adults, enjoys building meaningful relationships, working independently in the field, and collaborating with an experienced and supportive team.
What You’ll DoAs a member of our care management team, you will:
- Provide comprehensive care management and ongoing support to older adults living in the community
- Conduct regular in-home visits and assessments throughout your assigned territory
- Advocate for clients and help them navigate healthcare, housing, financial, and social service systems
- Assist clients in accessing benefits, entitlements, and community resources
- Develop individualized service plans and monitor client progress
- Collaborate with family members, healthcare providers, and other members of the client’s support network
- Maintain timely and accurate case documentation
- Participate in weekly team meetings and case conferences
- Contribute to program development and continuous quality improvement initiatives
Qualified candidates will possess:
- LMSW or LCSW
- Experience working with older adults, care management, case management, or related human services fields
- Knowledge of Medicare, Medicaid, Social Security, senior housing, and community-based services
- Excellent interpersonal, communication, and advocacy skills
- Strong organizational, documentation, and time-management abilities
- Ability to work independently while remaining an active member of a collaborative team
- Proficiency with Microsoft Office and electronic documentation systems
This position combines field-based client visits with administrative work and team collaboration.
- Be comfortable conducting regular home visits throughout the service area (NYC, Westchester, Long Island, New Jersey)
- Have a valid driver's license and reliable transportation a plus
- Be willing to travel locally to meet with clients, families, and community partners
- Attend weekly in-person team meetings at our office

Hybrid position:
- Field visits throughout the service area
- Weekly in-office meetings - Queens, NY
- Administrative work completed remotely and/or in-office as needed
