West Side Federation for Senior and Supportive Housing (WSFSSH) seeks a Bilingual Case Manager to support low-income older adults and people with special needs living in affordable, supportive housing in New York City. In this on-site role, you will provide person-centered, trauma-informed case management, conduct intakes and assessments, develop care plans, and connect residents to medical, behavioral health, and community resources.
You'll advocate for resident dignity and independence, collaborate with an interdisciplinary team, and help foster a safe, inclusive, mission-driven community grounded in social justice.
