The Clinical Coordinator II at West Side Federation for Senior and Supportive Housing (WSFSSH) oversees resident-centered clinical and supportive services within community and social services programs. This role conducts psychosocial assessments, develops and monitors individualized service plans, and coordinates care for low-income older adults and individuals with special needs. The Clinical Coordinator II collaborates with interdisciplinary teams and external providers, offers crisis intervention, advocacy, and referrals, and ensures regulatory compliance and accurate documentation.
